The Traditional PPO health plan offered through the state of Indiana covers the same services as a Consumer-Driven Health Plan. Like the CDHP services are covered after the deductible is met.  The Traditional PPO has a lower deductible and higher premiums. Please note that the co-insurance on the Traditional PPO is a bit higher as well at 30%.

Don’t forget to take advantage of 100% first dollar preventive care services. It is important to visit a primary care physician for an annual physical which includes screenings for high cholesterol and diabetes, the two most prevalent conditions among our plan members. Preventive services are a smart way to invest in your wellbeing.

As a Traditional PPO subscriber, you will pay a biweekly premium and the PPO will charge a coinsurance for each service rendered (for example, an office visit). Once your deductible is met, you will pay 30% of the negotiated cost until your maximum out-of-pocket is met.
