Name Change

  • Completed application
  • Certificate of Liability with new name shown (can only accept Certificate of Insurance for boat dealer name change).
  • Pictures of new sign with new name.
  • Copy of Retail Merchants Certificate with new name.
  • Fee is $5.00 for the amendment, $5.00 for the new license and $2.00 for each plate registration.
  • Original bond or bond rider, with original signatures, showing new name.
  • Copy of articles of incorporation or articles of organization to match the name on the application.

Address Change

  • Completed application
  • Certificate of Liability with new address shown (can only accept Certificate of Insurance for boat dealer name change).
  • Pictures of building, lot and sign.
  • Copy of Retail Merchants Certificate with new address.
  • Bottom of application completed by local zoning board.
  • Fee is $5.00 for the amendment, $5.00 for the new license and $2.00 for each plate registration.
  • Original bond or bond rider, with original signatures, showing new address.

Emergency 911 Address Change

  • Certificate of Liability with new address shown (can only accept Certificate of Insurance for boat dealer name change).
  • Statement from Planning Commission showing 911 change.
  • Fee is $5.00 for the amendment, $5.00 for the new license and $2.00 for each plate registration.
  • Original bond or bond rider, with original signatures, showing new address.
