- Course Provider Expiration Date: December 31, 2016
If your license has expired and you wish to renew, please email pla9@pla.in.gov with your name and license number to request a renewal form. You will need to complete and return the form, proof of insurance, and the renewal fee and late fee.
*Online license renewal is no longer available for the past renewal period. Renewal forms were sent through U.S. Mail on November 1, 2012 for the January 1, 2013 expiration date. If you have not received a renewal form please contact our office as soon as possible.
