NOTICE TO REGISTERED CEMETERIES

The State Board of Funeral and Cemetery Service has reviewed the balance of the Consumer Protection Fund for Cemetery Maintenance pursuant to IC 23-14-48.5-7 and determined that payment into the fund will be required March 1, 2020 for sales made in the calendar year 2019.  Please refer to IC 23-14-48.5-5 or the online form to determine how you are to calculate your payment.   Your payment (by check or money order) should be accompanied by the Disclosure & Payment into the Consumer Protection Fund for Cemetery Maintenance form.  You may obtain that form online at https://www.in.gov/pla/3693.htm.   NO PAYMENT REQUIRED MARCH 1, 2019 FOR SALES IN THE CALENDAR YEAR 2018.
