As a result of recent cost cutting measures and continued efforts to ensure taxpayer savings during this tough economic climate, effective December 1, 2009, the Indiana Professional Licensing Agency (“IPLA”) will no longer be printing in the office and/or mailing the 4½" X 6¾" blue wall licenses and pocket cards to individual licensees as a courtesy free of cost. 

This policy applies to all license types (business, facility, and individual) and also applies to issuance of new and renewed licenses.  However, Indiana law requires those engaged in the practice of pharmacy to display their license/certification/permit/registration cards where they are practicing.   As such, newly licensed and renewing individuals as well as facilities will need to utilize the IPLA website to order the original blue license cards or download the free license cards. 

How to Receive Your New or Updated License Card(s)
Newly licensed individuals and facilities
will receive an e-mail notification of their license number and instructions on how to login at http://www.in.gov/pla/license.htm to order their new license cards.  (You may also check online at http://www.in.gov/pla/license.htm to see if your license has been issued).  Once logged in to order the license card, you are given the option to download the free license card for immediate printing and/or purchase the blue card to be mailed to you; either card will meet pharmacy posting requirements.

Renewing individuals and facilities must verify that their license has been renewed at http://www.in.gov/pla/license.htm.  If renewing online, please allow 24 hours (1 business day) for the license to be updated.  If renewing by mail, please allow 7-10 business days for the license to be updated.  Once you verify renewal, you may order a new license card online at http://www.in.gov/pla/license.htm.  You are given the option to download a free license card for immediate printing or to purchase a blue license card to be mailed to you; either card will meet pharmacy posting requirements. 

In order to login and purchase or download your physical license, for individual licenses, you will need both your license number and the last four (4) digits of your social security number; for facilities licenses, you will need the license number.  Your license number will serve as your user ID to log into License Express, and the last four (4) digits of your social security number will be your password (for facilities and businesses, your license number serves as both your user ID and password).  The License Express service is available 24 hours a day and 7 days a week and accepts Discover, MasterCard, and Visa.

If you choose to download the free license card, make sure you request a quantity ("1") and add it to the "shopping cart" just as you would if you were purchasing a card to be mailed to you; otherwise, your information will not merge into the free card.

Please note, Board of Pharmacy staff will NOT have the capability to print and/or mail licenses.  This activity can only be conducted online through the License Express system.  Also, posting and maintaining a current license is still a requirement to practice in the State of Indiana.  Failure to maintain proper credentials can lead to violations during inspection and potentially include formal Board discipline.
