FEES
HFA/RCA Application:
$100.00 application fee payable to the Professional Licensing Agency.
$50.00 Temporary permit fee, if desired.
Preceptor Certificate Application:
$50.00
Provisional License:
$100.00
Renewal of license:
$100.00 every two years, due by August 31 of even-numbered years.
Renewal of license that has been expired for up to three years:
$100.00 renewal fee plus $50.00 late fee
Renewal of license that has been expired for more than three years:
