  • Speech-Language Pathologists and Audiologists:
    1. Application/Issuance Fee: $150.00
    2. Renewal of License Fee: $100.00
    3. Renewal of License that has been expired for less than three years: $50.00 penalty fee plus all past due and current renewal fees
    4. Renewal of License that has been expired for more than three years: $250.00 (Refer to IC 25-1-8-6(d)(2)&(3)) - Please contact the board office for information.


  • Speech-Language Pathology Support Personnel:
    1. Support Personnel Application/Issuance Fee: $50.00
    2. Renewal of Support Personnel Registration Fee: $25.00


  • Clinical Fellowship Year:
    1. CFY Application Fee: $50.00
    2. CFY Transfer of Supervision Application: No Fee
