RENEWALS
Optometry licenses and Optometric Legend Drug certificates expire on April 1 of even-numbered years. The next renewal date is April 1, 2014.

CONTINUING EDUCATION 
Continuing education is also a requirement for renewal. Please see the specific information on continuing education requirements.

A licensee is not required to submit copies of their continuing education certificates at the time of renewal. Upon renewing online or signing the renewal application you swear or affirm under the penalties of perjury that you have completed the required amount of continuing education. After the renewal, a percentage of practitioners will be chosen in a random audit to submit copies of their continuing education certificates. If the practitioner does not provide copies of the continuing education certificates upon request, the Board will precede with disciplinary action against the licensee.

RENEWAL FEE
The fee for renewal of an optometrist's license will be $100.00.

The fee for renewal of an optometric legend drug certificate will be $20.00.

INDIANA UNIVERSITY FEE
Based upon IC 25-24-2-3, in addition to the biennial licensure fee, each licensed optometrist shall pay, at the time of payment of the biennial licensure fee each even-numbered year, an additional fee of thirty-four dollars ($34). The additional fee is paid to Indiana University to be used for the advancement of optometrical research and the maintenance and support of the department in which the science of optometry is taught at the university.

RENEWAL INFORMATION
Optometry License and Optometric Legend Drug Certificate renewal notices are sent approximately sixty (60) days prior to the expiration date.   Licensees with a valid email address on file will be emailed the renewal information.   Licensees that do not have a valid email address on file will be mailed license renewal information.   

ONLINE RENEWAL
You can renew your Optometry License and Optometric Legend Drug Certificate at www.pla.in.gov.

  • Go to License ExpressJust use your license number with an "A" at the end (ex: 18009999A) as your Login ID and the last four digits of your social security number as your Password.   
  • You can update your address, telephone number and email address as part of the renewal process.
  • Pay your renewal fee(s) by using your Visa, Mastercard, or Discover credit or debit card.  There are access and credit card fees associated with renewing on-line.  No part of these fees come to IPLA.

Failure to renew a license on or before the expiration date automatically renders the license invalid without any action by the Board.  

LATE RENEWALS - PENATLY FEES
The Indiana If a licensee does not renew online or submit his renewal application on or before the expiration date the license automatically becomes invalid.  An optometry license may be renewed up to three (3) years by paying a late fee of $50 in addition to the renewal fee of $134.00 (Renewal fee and Indiana University fee) and completion of the continuing education.

The optometric legend drug certificate may be renewed by paying a late fee of $50 in addition to the renewal fee of $20.00.

After three (3) years of expiration an optometry license may be reinstated by completing the following:   

  • Renewal Application.
  • Payment of the renewal fee ($134) and reinstatement fee equal to the initial application fee ($200).  Total Fee:  $334.00
  • Completion of continuing education based upon the time period the license has been expired.  You will need to submit copies of the continuing education certificates.
  • Personal Appearance before the Board.
  • At the time of the personal appearance, the Board will determine whether remediation and/or additional training is required based upon the time the license has been expired.  This may include retaking the examination administered by the National Board of Examiners in Optometry and/or the Law Examination administered by the Board.

INACTIVE LICENSE
The Board may classify a license as inactive if the Board receives written notification from a licensee stating that the licensee will not maintain an office or practice optometry in Indiana. The renewal fee for an inactive license is one-half (1/2) the license renewal fee ($50.00) set by the Board. The holder of an inactive license is not required to fulfill continuing education requirements set by the Board.

If a licensee renews his/her optometry license on inactive status they are not eligible to renew their optometric legend drug certificate.

TO ACTIVATE AN INACTIVE LICENSE
The Board may issue a license to the holder of inactive license if the applicant:

  • Submits a renewal application;
  • Payment of the current renewal fee ($100.00);
  • Payment of the current reinstatement fee ($50.00); and
  • Submits copies of certificates of continuing education required by the board for each year, or portion of a year during which the applicant's license has been classified as inactive.

Please provide your name, full mailing address, license number and social security number with your request and be sure to state that you are requesting to reactivate your inactive chiropractic license.

LICENSE CARDS
The Indiana Professional Licensing Agency no longer automatically mails updated license cards when your license is renewed. You can determine if your renewal has been received and processed by accessing our free Online Verification Search. Please allow 24 hours (1 business day) for your license to be updated. Once you verify that your license has been renewed, you may order a new license card online at Services.IN.gov/License Express. You will be given the option to download a free license card for immediate printing or to purchase a blue license card to be mailed to you.  

ADDRESS CHANGE
You are required to notify the Board of any address changes for your optometry license and optometric legend drug certificate. Please notify the Board of your change of address by calling our office at (317) 234-2054 or email us at pla8@pla.in.gov or send a FAX to (317) 233-4236 to report a change of address. You may also submit your address change in writing to the:

NAME CHANGE
You may have the name changed on your licensure records by submitting a copy of a name change document such as a marriage certificate, divorce decree, social security card, or court papers.
