VERIFICATION LINE & FREE ONLINE VERIFICATION
The Indiana Professional Licensing Agency will verify the status of a license by calling (317) 234-2054 or you may obtain basic information on a practitioner's license for free by accessing our free online verification search.

VERIFICATION OF LICENSURE TO ANOTHER STATE
This is a verification of an Indiana license to another state.  The state of Indiana is now providing a digital certification of the license that you can download and email to your state board.  This is an official statement of the basis of licensure and includes the practitioner's name, profession, license number, status and any other conditions/restrictions of the license. Click here to order your digital certification. 

LICENSE CARDS
Initial and renewed license cards may be obtained at License Express. You may purchase a blue license card to be mailed to you or download a free license card for immediate printing.

Wall Certificates - The Indiana Professional Licensing Agency is no longer providing large 11" x 13" wall certificates. If you wish to order a large wall certificate from our vendor, the contact information is below:

REQUIREMENT TO DISPLAY LICENSE
Indiana Code 25-24-1-12 requires that an optometrist who is granted a license shall display it in a conspicuous part of the office in which the licensee practices optometry.  Whenever practicing the profession of optometry outside of, or away from, the office or place of business, the licensee shall make available to each patient or person fitted with ophthalmic materials the licensee's name, office address and number of the license.  

Therefore, you must either download the free license card or purchase a blue license card to post. IPLA staff cannot print license cards to be mailed or for walk-ins to our office.  

CONSUMER COMPLAINTS
Complaints against a practitioner are investigated by the Office of the Attorney General - Consumer Protection Division. Information concerning the filing of complaints may be obtained by calling 1-800-382-5516, (317) 232-6201 or (317) 232-6330 or at their website, www.in.gov/attorneygeneral/2434.htm.
