NEXT RENEWAL DEADLINE OCTOBER 31, 2013

 

In an effort to increase agency efficiency, the Indiana Professional Licensing Agency (IPLA), with the approval of the Medical Licensing Board (MLB) under IC 25-1-5-4(k), has extended the  physician renewal deadline from June 30 odd years to October 31, odd years.   

 

As a result, your current physician license and controlled substance registration (CSR) will now expire on October 31, 2013. 

 

When available, please complete the on-line SURVEY and RENEW your Medical license and all CSR's on-line by clicking on https://mylicense.in.gov/egov It is important to complete the Indiana State Department of Health's Survey as it will be used to determine areas of need in the future.

IT’S QUICK & EASY. It just takes minutes and your renewal will be processed and available for verification the next day. You can renew 24 hours a day, 7 days a week. You may pay by using your Visa or Master Card credit or debit card on our secure server. There are access and credit card fees associated with renewing on-line. No part of the access/credit card fees come to IPLA.

Renewal fee is $200.00 if renewed on or before October 31st. If renewed after October 31st a late fee of $50.00 will be due in addition to your renewal fee.

Pocket cards are no longer issued at renewal time.  Once you renew your license you may elect to print a free pocket card or purchase one.  Facilities may view our website http://www.in.gov/pla to verify your license.

All documentation submitted to our office should include your name and license number.

Renewal Positive Response: If you have a positive response to any of the renewal questions, you must submit a positive response statement via email to renewal3@pla.in.gov.  Your license cannot be renewed until this information is received and, upon further review, additional information may be requested by the Board.

Address Changes: You may also update your address and phone number on your medical license as part of the on-line renewal process. CSR address changes must be submitted to our office via e-mail or by mail. If you need to change your address after you have renewed your license, please send us an email with the change information and we will change it for you.

Name Changes: You may change your name on your medical license by faxing or mailing a legal document to our office. Acceptable documents are a marriage certificate, divorce decree, court document changing name. You may still renew on-line if you have a name change. Please send us the documentation and allow for one week to receive and process your change. You may then go on-line to renew and your pocket card should be issued in your new name.

Inactive Status: If your license is currently active and you wish to renew on inactive status you will not be able to renew on-line, you must contact our office and request an active to inactive renewal form. If your license is currently on inactive status and you wish to renew to active status you must also request a renewal form to come out of inactive status. To renew on inactive status you will be required to pay ½ the renewal fee or $100.00 if you renew on or before October 31st. Any license renewed on inactive status after October 31st will be required to pay the $100.00 renewal fee as well as a $50.00 late fee.

Expired License: If your license has been expired for three (3) years or more, you cannot renew online.  Additional items will be requested to renew this license, such as NPDB report, verifications from any state that you are/have been licensed in and a work history.  You may request an application by emailing pla3@pla.IN.gov
