INSTRUCTIONS & INFORMATION
FOR COMPLETING AN APPLICATION FOR
DIETITIAN CERTIFICATION

Please note that certification for a dietitian in Indiana is not mandatory. It is a title protection for individuals who wish to use the title "Certified Dietitian" or the credentials "C.D.".  You may practice dietetics / nutrition therapy services without state certification but cannot call yourself a "Certified Dietitian" unless you are certified with this state.

  1. APPLICATION
    1. Please type or print the application neatly with dark-colored ink.
    2. Include two (2) photographs, approximately 2 1/2 x 3 1/2 inches, head and shoulders view, black and white or color, of professional quality.
    3. Submit official transcripts from your college and/or university from where you received your degree. Note: Please have your official transcripts forwarded directly from the school to:

    4. Official documentation indicating any legal name change, if your name differs from that on any of your documents. Acceptable documentation is a copy of your marriage license or divorce decree.
    5. Document your pre-professional experience:
      1. If you are registered with the Commission on Dietetic Registration (CDR), please enclose a copy of a current registration card that indicates the period of registration and expiration.

        OR

      2. If you are not registered with the CDR, you must complete the attached Affidavit of Experience- Dietitian Intern Training Program form.
    6. List all states in which you hold or have held any type of license, certificate or registration for a regulated health profession. Include numbers, dates of issuance, and current status of all licenses.
    7. Answer truthfully the yes and no questions on the application. Attach information explaining any "yes" answer in its entirety.
    8. Submit the application and the documentation mentioned above, along with a check or money order in the amount of twenty dollars $20.00 (THIS FEE IS NON-REFUNDABLE) made payable to the Indiana Professional Licensing Agency, to:

    II.      VERIFICATION OF LICENSURE

You must submit the VERIFICATION OF STATE LICENSURE FORM to each state in which you hold or have held a license, certificate or registration in any regulated health profession. Please have any verification sent directly to the Indiana Professional Licensing Agency.

  1. EXAMINATION

    The Commission on Dietetic Registration (CDR) has contracted with the American College Testing Program (ACT) to administer the Registration Examination for Dietitians. The examination is computerized and currently given year round, Monday through Friday.

  2. THE FAIR INFORMATION PRACTICE ACT

    In compliance with IC § 4-1-6, this agency is notifying you that you must provide the requested information or your application will not be processed. You have the right to challenge, correct, or explain information maintained by this agency. The information you provide will become public record. Your examination scores and grade transcripts are confidential except in circumstances where their release is required by law, in which case you will be notified.

    Your social security number is being requested by this state agency in accordance with IC § 4-1-8-1. Disclosure is mandatory, and this record cannot be processed without it.
