Limited Temporary Permits
Applicants will need the following documents:
- Application for a Limited Temporary Permit To Practice Chiropractic
- Information and Instruction Sheet
- Indiana Board of Chiropractor Examiners Licensure Statute and Administrative Rules. A compilation from the Indiana Code and Administrative Code.
- Indiana Professional Licensing Agency Statute. A compilation of the statutes governing professions regulated by the boards and commissions under the Indiana Professional Licensing Agency.

INFORMATION
A limited temporary permit may be issued to a person who is licensed to practice chiropractic by a board or licensing agency of another state or jurisdiction.
A limited temporary permit shall be limited to a specific activity, function, series of events, or purpose and to a specific geographical area within the state, which limitations shall be stated on the temporary permit.
A limited temporary permit is valid for a nonrenewable period of not more than thirty (30) days.
REQUIREMENTS FOR THE ISSUANCE OF A LIMITED TEMPORARY PERMIT
A person licensed to practice chiropractic by a board or licensing agency of another state or jurisdiction may apply for a limited temporary permit by submitting an application with the following information:
- The purpose for applying for a temporary permit.
- The location where chiropractic services will be provided by the applicant.
- The activity, organization, function, and event with regard to which the chiropractic services will be provided by the applicant.
- The applicant's practice address and telephone number.
- One (1) recent passport-quality photograph of the applicant, taken within eight (8) weeks prior to filing the application for a temporary permit.
- Verification that the applicant is a graduate of a school or college of chiropractic accredited by the Accreditation Commission of the Council on Chiropractic Education (CCE).
- Verification from other states where the applicant is licensed stating that the applicant's license is current and in good standing and is not or has not been the subject of disciplinary action.
COST OF LIMITED TEMPORARY PERMIT
Applicants must submit a fifty dollar ($50.00) application fee, made payable to the Indiana Professional Licensing Agency. This fee may be submitted by cash, check or money order. We cannot accept payment by credit card.
