Complete and File with the Board of Law Examiners:

  1. The original and one copy of:
    1. Attorney Listing Cover Page
    2. Exhibit Attachment listing all attorneys
    3. Filing fee of $10 for each attorney added, make check payable to Board of Law Examiners
  2. The original and one copy of:
    1. Clerk of the Supreme Court Certification
    2. Exhibit Attachment listing all attorneys (this is the same document as 1c above, but must also be included with the Clerk’s Certification)
    3. Fee in the amount of $1.00 for each attorney added on Exhibit Attachment, make check payable to Clerk of the Supreme Court
  3. The original and one copy of:
    1. Disciplinary Commission Certification
    2. Exhibit Attachment listing all attorneys (this is the same document as 1c above,   but must also be included with the Disciplinary Commission Certification)
