Indiana law, IC 8-1-2-16, requires utilities to file reports annually on or before April 30. The Commission maintains the collection of annual reports for nearly 400 utilities. Certain utilities, such as resellers of interexchange services, WATS, radio common carriers and cellular/wireless companies are not required to file annual reports due to deregulation or a utility's withdrawal from Commission jurisdiction.
Due Date
Annual reports must be completed in full and received by the Commission on or before April 30, 2011. ABSOLUTELY, NO EXTENSIONS.

Electronic Annual Reports
You may obtain a copy of most annual reports by downloading the appropriate version for your utility. Listed below are the various types and classes of annual reports. In most instances, whatever class report you filed in previous years will be the one to file for 2010. Be sure to save a blank copy of the report to an Excel file before you fill out the report. If you are not sure which report is appropriate or have other questions or problems regarding annual reports you may contact the IURC Natural Gas Division at 317-232-0037. Natural gas utilities may direct inquiries and file completed annual reports via email at Natural Gas Division. You are required to mail in a hard copy of the Certification page with a notarized signature.

Option 1 (Preferred):
If you choose to submit electronically, attach the utility’s report(s) in either Excel or PDF format to an email and send to the Natural Gas Division at Natural Gas Division or click on the Electronic Filing System link https://myweb.in.gov/IURC/efs/. If the State of Indiana Notary seal is not visible, you will be required to mail in the original certification page.
Option 2:
If you choose to use a carrier, or the U.S. Postal Service, we ask that you DO NOT BIND OR STAPLE any report forms, submit Originals only (NO photocopies), and mail to:
