During the 2011 Session of the Indiana General Assembly, the General Assembly passed Public Law 193-2011 [House Bill 1210] pertaining to abortion matters. Public Law 193-2011 requires the Indiana State Department of Health (ISDH) to post Internet web site links concerning materials described in the statute on the ISDH’s web site.
Senate Enrolled Act 371 (2013) requires the ISDH to develop an informed consent brochure and post on the ISDH web site. The statute also requires the ISDH to develop forms certifying that abortion information has been provided to the pregnant woman seeking an abortion.
This web site is provided by the ISDH in compliance with statutory requirements. This site provides the informed consent brochure and links to external sites that provide information about abortions. This information is intended to be informational. The Indiana State Department of Health does not endorse nor is responsible for the content provided in external links. References to legal requirements in may refer to legal requirements in other states and not necessarily be applicable in Indiana.
ISDH Abortion Informed Consent Brochure
The ISDH was required by Indiana statute to develop an informed consent brochure and post on the department’s web site [Senate Enrolled Act 371 (2013) - IC 16-34-2-1.5]. The abortion provider is required to provide the pregnant woman with a color copy of the brochure [IC 16-34-2-1.1(a)(4)]. The ISDH Abortion Informed Consent Brochure is the statutorily required brochure.

(PDF | Word) Abortion Fetal Ultrasound and Heart Tone Certification Form
Before an abortion is performed, the abortion provider must perform a fetal ultrasound and fetal heart tone procedure. The provider must enable the pregnant woman to view the fetal ultrasound image and hear the heartbeat of the fetus if the fetal heart tone is audible [IC 26-34-2-1.1(b)]. The purpose of this form is to document the opportunity to view the image and hear the heart tone.

Indiana 211 Partnership, Inc. (IN211) is a nonprofit organization dedicated to building the 2-1-1 system for Indiana. The goal is to create a seamless network of information and referral services that enables anyone in Indiana in need of human services to have quick referrals to those who provide them by dialing 2-1-1. Additionally, aggregate data will be collected to assist communities in assessing needs and allocating resources.
The Indiana Utility Regulatory Commission (IURC) recognized the Partnership as the proper administrator for the 2-1-1 dialing code on February 20, 2002, and issued a final order on June 17, 2004. The Indiana General Assembly passed legislation supporting 2-1-1 that became effective July 1, 2004 (I.C. 8-1-19.5). IN211 is actively working to implement 2-1-1 service in order to serve all Hoosiers through linked IN211 Centers.
