New Law Concerning Immigration Status for Participants and Applicants Who are 18 Years and Older The Indiana State Department of Health is now required to verify that any individual 18 years or older, who is currently on the Children’s Special Health Care Services program, or who is applying for the Children’s Special Health Care Services program, is either a U.S. citizen or a qualified alien. The following form must be used to verify the immigration status, and can be signed by the applicant’s parent or guardian.
