An International Travel Clinic provides pre-travel individualized risk assessments and education about health concerns to patients and provides necessary prophylaxis and vaccinations (including yellow fever vaccine) as recommended or required for travel abroad to a given destination. Travel immunization, prophylaxis, and other health concern guidelines are available by destination at http://www.cdc.gov/travel.
Yellow fever vaccine is the only required vaccine for travel to some countries. Some countries not designated as yellow fever risk areas require vaccination documentation if the traveler is arriving from a risk area.
Providers who wish to become International Travel Clinics and administer yellow fever vaccine must be approved by the Indiana State Department of Health (ISDH). Any physician (M.D. or D.O.) licensed in Indiana is eligible to become a certified yellow fever vaccine provider. The process for becoming a certified yellow fever vaccine provider is as follows:
Upon receipt of your application, ISDH staff will verify that all information is complete and accurate. Your approval to administer yellow fever vaccine will be submitted to Sanofi Pasteur (manufacturer of yellow fever vaccine), and orders can be placed directly through this manufacturer. Note that yellow fever vaccine is shipped only to certified providers at the address indicated on Yellow Fever Vaccine Certified Provider Application Form.
