Thank you for registering for an upcoming HMIS training.  Once your registration has been approved you will receive an email with instructions for attending the webinar.  Please make sure you registered with a valid email address or you will not get the registration information.  Below you will find all of the material you need for most HMIS webinar.

Training Material

New User Training Manual - This document contains an outline of the New User Training as well as detailed instructions for processing intakes, discharges, and updating client records. 

Data Definitions - A document explaining all of the fields that show up at intake and discharge.

Intake Checklist - A flowchart for using HMIS.  Coming Soon!

Paperwork for New Users - New User Information, Code of Ethics

If you have never used the HMIS system you will first need to get a User Name and Password and review of the security requirements of the program. 
Please fill out a copy of the User Code of Ethics, have your executive director sign off and fax it to 317-232-7778 to the attention of HMIS Staff.  In addition, please be sure to read and post the Notice of Privacy Practices. 

If your agency is new to HMIS you would need to contact the HMIS Project Manager to get approval for using the system.  New agencies will need to fill out the Agency Partner Agreement and fax the signature page to the number above.

Additional Information

After registering for the training you should receive an automatic message with the Webinar link and conference call #.  If you do not get a confirmation email before the training date, it most likely means that you did not register with a valid email address (perhaps due to a typo) and you should contact the HMIS Trainer for verification that you registered correctly.
