IDOI provide instructions that assist individuals in preparing the application for a discount medical card program organization (DMPO) with all the information required by statute and facilitate expeditious processing of the application by IDOI. Please submit your package in a tabbed binder. Please identify the tabbed sections by the section number reference, e.g. II-2 would contain the By-laws and other legal documents.

Submissions that do not contain all the required information will be returned without review or approval.
