Report Description

The Annual Manifest and Biennial Hazardous Waste reports document the types and amounts of RCRA hazardous wastes that are generated, shipped off-site, treated on site and/or received from off-site.

Date Due:

  • Reports must be postmarked by March 2.
  • Extensions until April 1 may be granted. Send your request, via email, to "olqregulatoryreporting at idem.in.gov" and specify the reason for the request.

Who Must File and Which Report Must be Submitted

Small Quantity Generators Annual Manifest Report

  • Any facility that was a Small Quantity Generator (SQG) at least one month of the year, but was not a Large Quantity Generator anymonth during the year, must submit the Annual Manifest report.
  • Conditionally Exempt Small Quantity Generators (CESQG) that for any one calendar month generate more than 220 pounds or accumulate on-site at any time more than 2,200 pounds of hazardous waste will also be required to complete and submit the Annual Manifest report.

Large Quantity Generators Biennial Hazardous Waste Report or Annual Manifest Report

  • Any facility that was a Large Quantity Generator (LQG) at least one month of the year must submit either an Annual Manifest report OR a Hazardous Waste Biennial report depending on the year.

Treatment, Storage, Disposal Facilities Biennial Report or Annual Manifest Report

Any facility that was a permitted Treatment, Storage, or Disposal facility (TSD) during the report year must submit either an Annual Manifest report OR a Hazardous Waste Biennial report depending on the year. Facilities that are undergoing closure, but have not yet received final approval from IDEM must continue to report. If no waste was received during the year and the facility is not operating as a generator of any type, the ID form should be submitted.

If Your Facility is not an LQG, SQG, OR TSD

  • If your facility was not an LQG, SQG, or TSD facility in any month of the year no report is required.
  • However, if you received a reporting reminder in December you MUST complete and return the Handler ID form to the Office of Land Quality. This is necessary so that we can document that you were not an LQG or SQG during the report year and do not owe a report. Even if the business is no longer active the entire report is still required if the facility acted as an SQG or LQG in any one month during the report year.

Handler Identification Form

The Handler ID Form is required to be submitted by every LQG, SQG, TSD, and any other facility that received a reminder notice in December.

  • The ID form should be evaluated for accuracy, corrections made if needed, signed and submitted with the rest of the report.
  • When filing electronically the ID form should be sent after the electronic report is submitted.
  • The ID form is NOT the entire report.
  • If you did not receive an Indiana Hazardous Handler Identification Form you may request one from IDEM.

Report Forms

All Annual Manifest and Biennial Hazardous Waste reports should be submitted electronically.

  • The ARM Easitrak software must be used to file electronic reports.
  • If you submit electronically, the Handler ID form must still be sent in hard copy.
  • The Declaration of Electronic Reporting must also be submitted. This is found in the ‘reports’ tab of Easitrak.
  • The Handler ID form and the Declaration form should be sent at the same time and after you submit the report electronically.
  • The Easitrak Site Identification form (SI) must be completed on-line but should NOT be sent in place of the Indiana Handler ID form.
  • The instructions found at the Easitrak site are generic instructions for multiple states who use the software. You should read the Indiana specific instructions (below) for Easitrak and for the Annual or Biennial report before using the software.

Submitters with their own data systems and can create flat files may submit files in the specified format. Contact IDEM at (317) 233/0066, or via email at "olqregulatoryreporting at idem.in.gov" for a copy of the flat file specifications.

If you absolutely cannot submit electronically you may still submit in hard copy. The forms are available in both PDF and DOC fillable formats on the IDEM Forms page. The Handler ID Form is required in hard copy. This form is not on the forms page but is available upon request.
