Annual Report Form for facilities permitted under Rule 6
The Office of Water Quality has created a new state form for all permitted facilities to complete and submit to assist permittees in meeting the annual reporting requirements of Rule 6.
Rule 6 requires each permittee to complete and submit an Annual Report. Annual Reporting is an effective way of ensuring the Storm Water Pollution Prevention Plan (SWP3) is current, potential pollution sources are identified, and Best Management Practices and corrective measures are implemented, properly designed and functioning, and are being maintained.
The Industrial Storm Water General Permit annual reporting requirements can be found in 327 IAC 15-6-7.5 of the Rule. Click on 327 IAC 15-6 (Rule 6) and scroll to the bottom of page 34 to access the monitoring requirements.
- The Annual Report - State Form 54185 (available on the IDEM Forms page) is due no later than 365 days from submitting the Notice of Intent and must contain information obtained during the previous year of regulation. Subsequent annual reports shall be submitted no later than 365 days from the previous report in years 2 through 5.

Storm Water Discharge Monitoring Report
The Office of Water Quality has created a new state form for all permitted facilities to complete and submit to assist permittees in meeting the monitoring requirements of Rule 6.
Rule 6 requires each permittee to conduct annual storm water monitoring. Annual storm water monitoring is an efficient way to evaluate the effectiveness of the Storm Water Pollution Prevention Plan (SWP3). The Industrial Storm Water General Permit monitoring requirements can be found in 327 IAC 15-6-7.3 of the Rule. Click on 327 IAC 15-6 (Rule 6 and scroll to the middle of page 33 to access the monitoring requirements.
The Storm Water Discharge Monitoring Report (available on the IDEM Forms page) is due no later than 365 days from submitting the Notice of Intent. Subsequent annual storm water discharge monitoring reports shall be submitted no later than 365 days from the previous report in years 2 through 5. Each year of the permit, facility representatives are required to collect grab storm water samples from qualified rainfall (scroll to the Section Titled “Qualified Rain Events”). The samples must be sent to a lab and analyzed for any pollutant that has the potential to be present in a storm water discharge in addition to the following required 8 sampling parameters:
- pH;
- Oil and Grease;
- Nitrates plus nitrite nitrogen;
- Kjeldahl nitrogen, total (TKN);
- Phosphorous, total;
- Suspended solids, total (TSS);
- Chemical oxygen demand (COD);
- Carbonaceous bio-chemical oxygen demand (CBOD5)
