Eligibility requirements for a Cemetery Heritage sign can be found in the Cemetery Heritage Sign Application.
Eligible applicants should complete and return an application to Cemetery Registry Coordinator; Division of Historic Preservation and Archaeology, DNR; 402 W. Washington St, Room W274; Indianapolis, IN 46204.
IHB has established the following deadlines for Cemetery Heritage Sign applications:

IHB will send an invoice and text approval form after the application has been received and processed. The cost of the sign and shipping (if applicable) must be prepaid with completed approval form by this date or the application will be moved to the next deadline and delivery will be delayed.
Signs will be available approximately 4-6 weeks after the payment deadline.
