Potential Individual Taxpayer Questions

  1. What if I have not yet filed my individual tax return yet and all my paperwork has been destroyed or lost?
    You will need to contact the organizations or employers who provided them to you initially. They should have copies. For help, contact 317-233-5212.
  1. What if I live in an area affected by severe weather and I’m expecting a refund from the state – but, the refund hasn’t arrived yet?
    The department will work with you to discover the status of your refund and, if due, will work with you to get that refund to you. Contact us at 317-233-5212.
  1. What if I live in an area affected by severe weather and my refund check was destroyed before I could cash it?
    Taxpayers can request a replacement check by completing an Affidavit for Lost or Not Received Warrant form. Contact 317-233-5212 for assistance with obtaining this form.

  1. What if my business is in an area affected by severe weather, and I’ve lost my sales-tax records?
    We can assist you with those records. Call 317-233-5212 and explain the problem. Make certain you verify your current or temporary mailing address when you call.
  1. What if my business is in an area affected by severe weather, and I’ve lost my trust-tax coupons?
    We can send you replacement trust-tax coupons through the mail, or at one of our district-office locations in person. Select the closest District Office (Columbus or Clarkesville).
  1. What if my business is in an area affected by severe weather, and I’ve lost my payment-plan coupons?
    Taxpayers can get replacement payment-plan coupons through the mail, or by visiting any district-office location to receive them in person.

    Taxpayers in an area affected by severe weather can replace their active RRMC through the mail, or by visiting any district-office location to receive the RRMC in person.
