This page has been established to assist members of the media and other interested parties with inquiries regarding Fatality and High Priority Incidents investigated by the Indiana Occupational Safety and Health Administration (IOSHA). The following is not a complete list of all work-related fatalities in Indiana; however, those posted are within IOSHA jurisdiction. Information will be updated within two business days of IOSHA notification. Please find the Fatality and other High Priority Incidents organized by date and location of event below.
December 2010
- December 31, 2010: Chrysler (Seymour) Employee died from a fall.
- December 14, 2010: Green Tech Transfer & Recycling Station (South Bend) Employee was killed as a result of an explosion.
November 2010
- November 29, 2010: Kunce Brothers, Inc. (Cromwell) Employee was crushed by rear trailer wheels at a family farm.
- November 9, 2010: Industrial Contractors, Inc. (Edwardsport) Employee died of a possible heart attack.
- November 5, 2010: Jackson Township Fire Department (Helmsburg) Employee was pinned between a truck and a wall.
- November 4, 2010: Bosselman, Inc. (Hobart) Employee was pinned between service truck and semi trailer.
October 2010
- October 27, 2010: University of Notre Dame (Notre Dame) Employee fell from scissor lift that tipped over.
- October 27, 2010: Steele Rigging Corporation (Boonville) Employee fell through roof surface.
- October 20, 2010: ArcelorMittal Indiana Harbor (East Chicago) Employee was struck in head by leg of lift table.
September 2010
- September 29, 20010: Pilkington North America (Shelbyville) Employee was performing maintenance work on a conveyor when he became pinned.
- September 9, 2010: F & F Coatings (Kokomo) Employee became entangled in cables when the scaffold he was working on dropped from under him.
August 2010
- August 9, 2010: Bootz Industry (Evansville) Employee ejected and crushed under a forklift they were driving which overturned.
July 2010
- July 28, 2010: Burke Repo Outlet (North Webster) Employee involved in a traffic accident while test driving a motorcycle.
- July 23, 2010: Utili.Comm South (Terre Haute) Employee fell from a ladder while installing cable at a residence.
- July 20, 2010: Newmar Corporation (Nappanee) Employee struck head after being ejected from the back of a pick-up truck which hit a bump.
June 2010
- June 28, 2010: Trees Are Us (Mishawaka) Employee struck in head by a branch being worked on by trimmers.
- June 17, 2010: Crane Master (Richmond) Employee crushed under a crane which tipped over.
- June 1, 2010: Bulk Transport Corporation (Burns Harbor) Employee crushed by a forklift while he was working underneath.
May 2010
- May 26, 2010: G. C. Zarnes & Co. Inc. (Edwardsport) Employee fell ~40 feet from a suspended scaffold in an empty steel tank.
- May 26, 2010: Liberty Township Fire Department (Muncie): An independent plumber, working in a well, was fatally overcome by fumes. While attempting a rescue the plumber's helper and two volunteer firefighters were also overcome and hospitalized. The helper died later.
- May 23, 2010: Unilever (Hammond) Employee crushed by a metal cover he was lifting with a remote control crane.
- May 11, 2010: Hixon Home Improvement (Porter) Employee fell down stairs and struck their head while moving a chest freezer. Employee died May 18.
- May 5, 2010: Carmel School Corporation (Carmel) Employee burned in a lawnmower explosion. Employee died May 8.
April 2010
- April 30, 2010: Moonrock, Inc (Burns Harbor) Employee struck by a car during road construction. Employee died May 2.
March 2010
- March 26, 2010: Keystone Builders (Warsaw) Employee fell 30 feet from a roof and broke their neck.
- March 16, 2010: SM&P Utility Sources (Valparaiso) Employee struck by a car while spray painting road for utility. Employee died March 18.
- March 16, 2010: Walsh Construction (Gary) Employee struck by car while patching potholes on highway.
- March 15, 2010: Kroger (Franklin) Employee crushed under pallets of water which had fallen. Employee died March 20.
- March 8, 2010: Mosher Enterprises (Monon) Employee struck by a passenger train while driving over railroad tracks in a company truck.
January 2010
January 16, 2010: Speedy Petroleum (Indianapolis) Employee shot during a robbery.
January 13, 2010: Talls Tree Timber Co. (Martin County) Employee died while trapped in a log skidder which fell into a river in cold weather.
January 7, 2010: Beta Steel (Portage) Employee died of blunt force trauma after a steam explosion in a furnace room, four other employees were injured, two requiring hospitalization.
January 6, 2010: Thiesing Veneer Co. (Mooresville) Employee fell into a hot water tank, scalding his back and legs. Employee died January 15.
January 1, 2010: Rescare Northern Indiana (Auburn) Employee shot by an ex-boyfriend.
December 2009
- December 29, 2009: Peterson Grain (Decatur) Employee fell 80 feet into an empty grain silo while performing maintenance.
- December 6, 2009: City of Anderson - Street Department (Anderson) Employee struck by a vehicle while cleaning up debris from a traffic accident.
November 2009
- November 21, 2009: Village Pantry (Indianapolis) Employee shot during a robbery
- November 18, 2009: Brown Transport, Inc. (Carthage) Employee electrocuted while exiting the cab of a dump truck which had made contact with overhead power lines
- November 18, 2009: Topp Industries (Rochester) Employee crushed by 1,000 pound machinery part which slipped off mandrill during removal
- November 16, 2009: John Collier Logging, Inc. (New Castle) Employee struck on head by a limb while cutting trees
- November 11, 2009: Horner Industrial Services (Indianapolis) Employee caught in machinery while doing repair work

- August 5, 2009: Steeel Dynamics (Jeffersonville) Employee entered a confined space and was overcome by nitrogen
- August 3, 2009: Danville Chrysler Dodge Jeep (Danville) Employee fell from golf cart and struck head
