Indiana Child Protection Service History Checks

Important Notice Regarding Upcoming Procedural Change

In the last Spring/early Summer of 2018, the Indiana Department of Child Services will move to an online submission portal for all submissions of the Indiana Child Protection Service (CPS) checks, Expanded Child Protection Index (CPI) checks as well as request by Out of State Child Welfare Agencies regarding information to assist with that state’s ongoing investigation/assessment. At a yet to be determined date, DCS COBCU will no longer accept paper form request or request for information by e-mail or fax. 

In this new process, the requesting party will initiate the process for a CPI/CPS search through the online submission portal. Each requesting agency will need to register with DCS as part of the portal.   After being registered, the requesting party will enter data for the applicant.   Once the requesting party portion is complete, an e-mail will be sent to the applicant of the check requesting they log into the portal to complete the remaining fields as to their personal information. During each step of the process both the requestor and the applicant will receive e-mail communications regarding the status of the check. Out of State Child Welfare Agencies will also be required to use the online submission portal but the submission process will be altered to meet the requirements of the unique request type.

Those organizations who make numerous requests annually from DCS will also be able to log into the system and see all previous requests that they have submitted in one location, simplifying the process. Upon completion of the individual checks, an e-mail will be sent to the applicant and the requestor who will then log into the electronic portal to view the request as well as print hardcopies of the results, if appropriate.

The intent of this portal is to assist in eliminating duplicate, incorrect, and incomplete submissions to DCS, which has historically been a barrier to efficient processing and timely requests. At a future date, all CPI and CPS checks as well as out of State Child Welfare Agency request and other state of Indiana agencies that access DCS information will be required to be submitted and complete this work through the electronic portal and paper forms will no longer be accepted.

Local DCS office’s contact information

Any CPS Check that requires information on an applicant who resided in Indiana prior to 1998 must be sent to the local DCS Local Office in the county or counties that the applicant resided in during that time period.  Any local DCS office can also complete a Statewide CPS checks for dates 1998 to through current.  Statewide CPS check requests can also be sent to the Central Office Background Check Unit (COBCU) for periods after 1998.  CPS check requests can be sent to the DCS local office where applicant lives or works or the DCS central office.

Please make sure you have filled out the form completely to ensure it can be processed. Incomplete or incorrectly completed forms will be returned unprocessed.  There is no cost for this search.  All forms require a valid signature of release.
